Definitely fake. I think this woman is black, so this is probably a stupid "black people like Trump" thing that someone is trying to spread. I hope it doesn't work, but sometimes this garbage can change some minds.

Definitely, unless that person just has the least responsive skin in the world. A ballpoint pen is going to do more damage than you see there.
